General Instructions for Seeing a Specialist

STEP 1:  Pick up the phone and CALL!

  1. Call one of the doctor’s offices listed on the handout we provided and ask for an appointment.
  2. Tell the person scheduling your appointment what you are being referred for (this is written on the referral form we gave you).

STEP 2:  Use this space toWRITE down the date and time of your appointment and save this paper!

DAY:  _______________________  DATE:  ________________  TIME:  _______________

STEP 3:  GO to your appointment!

  1. When you go to your specialist appointment, make sure to bring:
    • ID card 
    • Insurance Card
    • Medication list/pharmacy information
    • REFERRAL PAPERWORK
    • Copy of your x-rays
