General Services
These are medically necessary procedures that relieve pain, eliminate infection, and prevent disease.
- Exams and x-rays
- 3- or 6-month teeth cleanings
- Deep cleanings
- Basic fillings for cavities
Specialty Services
Optional procedures, not offered at most other FQHC dental clinics, which restore function and esthetics.
- Complete and partial dentures
- Root canal therapy
- Crowns and bridges
- Night guard
Determining eligibility for the Sliding Fee Program has not changed; it is based on household income and family size compared to Federal Poverty Guidelines.
Proof of income is required to participate in the Sliding Fee Program. Verification of income can be any of the following:
- 1040 forms for the previous calendar year
- Copy of the previous year’s W-2 form
- Current pay stub
- Unemployment check stub
- Social security check or award letter
- Letter from employer stating earnings
