EONS (Education, Outreach and Navigation Services)

The Education, Outreach and Navigation Services (EONS) Program at Philadelphia FIGHT provides comprehensive, patient-centered, whole-person healthcare navigation and care coordination services to those living with or at risk of contracting viral hepatitis, HIV, sexually transmitted infections, and those experiencing complications of unstable housing, poverty, substance use disorder, and other factors contributing to healthcare disparities.

The EONS team provides the following services:

  • Rapid HIV and hepatitis C (HCV) testing at community health events and with partnering social and health service organizations
  • Community-based education sessions regarding the importance and availability of primary care and subspecialty health care including HIV, HCV, dental, PrEP/PEP, medications for addiction treatment (MAT), STIs, and cancer screening
  • Individual in-person counseling sessions for people incarcerated in Philadelphia and living with HIV, HCV, and receiving or considering MAT care.  Specialized care is also available for returning citizens.
  • Patient navigation and care coordination for individuals living with HIV, HCV, and receiving MAT and harm reduction services 
  • Ensuring individualized support while in care at FIGHT for HIV, HCV and MAT medication adherence, treatment courses, and other supportive health services.
