Care Outreach services are designed to help those experiencing difficulty in accessing care overcome the challenges that confront them, so that they are able to take care of their health. Care Outreach staff is available to contact clients who have been unable to keep appointments, accompany clients to emergency rooms, specialist visits, mental health visits, or substance abuse visits when the clients need the extra support. Care Outreach workers make home visits when needed.

Clients who give us permission in advance to contact them will find that our goal is to make sure that no one is left out of care because life challenges seem to difficult to overcome. When that happens, our staff is there to help.

Care Outreach also has a special emphasis on prison outreach. Those leaving the Philadelphia county jails often face especially complicated challenges in accessing health care.
